A proof of address is required for all families participating in the Adams 12 Choice application process. They are also required by the Colorado Department of Education. Proofs of residency that are accepted include utility and cable bills, mortgage statements, purchase contracts, lease agreements, and renters' insurance. We will offer support for students on IEPs with mild/moderate needs and will have learning specialists on staff. Students will receive support based on their IEP through small group guided instruction (elementary), some specialized classes (secondary) and access to staff through the Learning Center. Given the nature of online learning, Five Star Online Academy may not be a good fit for all students; we would encourage parents to consult with their school's SSS coordinator prior to applying to Five Star Online. Spaces are subject to capacity and staffing resources.

We will not have programming for students with Significant Support Needs at the Five Star Online Academy.

DISTRICT NOTICE TO PARENTS OF STUDENTS WITH DISABILITIES:

The Choice program is open to students with disabilities. Students with disabilities placed in a Significant Support Needs, Affective Needs or Autism Spectrum Disorder Program may apply to any program listed in the special education section on the Choice website. Spaces are subject to capacity and staffing resources. If no programs are listed below, the district does not have the available space or staff to accommodate students from outside a school’s attendance area or outside of the Adams 12 boundaries with existing resources.

Five Star Online Academy offers English Language Development (ELD) services for English Language Learners; however, services and student support may look different than that of a traditional brick and mortar school. Five Star Online Academy works collaboratively with parents and guardians during the application process to determine the most supportive learning environment.

Beginning with the 2026-2027 school year, we will offer grades 6-12.

No. Once your student is enrolled at the Five Star Online Academy, they will remain enrolled throughout all subsequent grade levels, excepting the following:

• If subsequent to the submission or approval of your Choice application, expulsion, the recommendation for expulsion, or engagement in conduct which would support expulsion occurs, the approval for Choice becomes null and void.

• The District may rescind approval of a student’s choice application at any time, including school years subsequent to the initial application, if it determines that the application included material misstatements or omissions.

Enrollment is a commitment for an entire academic school year. You would have the opportunity during the district's regular Choice Application window to request to return to your boundary school or another school open to choice for the following school year. This occurs on an annual basis.
